How a Rootworth online intake works.

This is the actual sequence — who reads your intake, where your records live, what it costs, and how to begin.

The practitioner and the clinic.

Michael Woodworth, L.Ac. — California-licensed acupuncturist (License #AC10818). Classically-trained herbal specialist. Twenty-five-plus years of clinical practice. He reads every intake personally.

Rootworth is the online herbal arm. The clinic that holds the records is Makari Wellness — Michael’s in-person practice, running since 2005. One practitioner, one HIPAA-compliant system, two reaches of the same practice.

How we treat: modern delivery for classical medicine.

Rootworth exists at the intersection of two commitments: honoring classical herbal medicine and making it actually work for people in the modern world. That intersection means using powder extract formulas.

Why powder extracts.

Compliance is everything. The biggest obstacle to herbal treatment success has always been consistency. A patient prescribed raw herbs faces the reality: boiling herbs for an hour, several times a week, indefinitely. Life happens. Schedules shift. Compliance drops. Results plateau or disappear. With powder extracts, there’s no barrier. Mix, drink, move on. Consistency of dosage becomes possible — and consistency of dosage is what actually drives change in the body.

Quality and practicality. Raw herbs are more potent, and for severe acute conditions, they’re necessary. But clinically, raw herbs create a supply chain problem. They spoil. They have short shelf lives. Powder extracts, sealed in airtight jars, maintain integrity for years.

The quality hierarchy matters. Raw herbs are the highest grade — visually inspected, whole plant material. Powder extracts are the second highest: concentrated from quality plant material, standardized, shelf-stable. Below that are mass-produced tea pills — cheap, low-quality, and often clinically ineffective. We use the second-best option because it’s the one that actually works in real life.

Customization at scale.

Once we have your intake, we use concentrated extracts in combination to design formulas specifically for your constitution, your presentation, your life. This is classical herbalism delivered through modern logistics.

It’s not a compromise. It’s the only way to make classical medicine actually reach people.

What $150 actually gets you.

In private integrative medicine, an initial consultation alone typically runs $150–300 — before you see a single herb. Here, that $150 is the whole first chapter:

  • The intake examination. Michael reads your full questionnaire against the classical diagnostic framework — mapping where the body is, what pattern is driving your presentation, and what the formula needs to do.
  • Your pattern diagnosis and first custom prescription. Not a template. A formula written for you, for this pattern, at this stage.
  • Your first seven to ten days of herbs, shipped. High-quality powdered herbal extracts — $50–70 in herbs and shipping, included.
  • Your second appointment — the first follow-up re-exam. After you have taken the first formula, Michael evaluates how the body is responding and adjusts the prescription if needed. That re-exam is included in your initial fee.

One investment. Four deliverables. The full arc of your first course of treatment.

After the first follow-up: refills are herbs + shipping only. Formula revisions include a $50 re-exam fee. The $150 charge occurs when Michael begins reading your completed questionnaires. Charges appear on your statement as Makari Wellness.

Why herb cost varies. The cost of a formula comes down to two factors: the ingredients it contains and its total weight. Ingredients matter — some herbs are costlier than others — but the primary driver is weight. Weight determines how many days of formula you receive, and that is governed by your daily dosing. In short: your daily dose sets how much herb you go through, the number of days sets the total weight, and the weight sets the cost. You pay for the medicine actually dispensed — nothing more.

How to begin.

Step 1 — create your portal account. The button below sends you to the Makari Wellness clinic portal. Create your account there. Your intake, chart, and formula history all live in this account.

Step 2 — choose your service. Inside the portal you’ll select the option that fits where you are:

  • New to the practice? Choose the initial consultation. This opens the full history-and-intake questionnaire that sets up your file for your first diagnostic review.
  • Returning patient? Choose the re-exam. This opens the re-exam questionnaire to refill or adjust your existing formula.

Step 3 — pick any appointment time. Choosing a service prompts the system to ask you to schedule a time. The specific time doesn’t matter — there is no live appointment to attend. It’s simply the step that unlocks the correct questionnaire. Pick whatever slot is convenient and continue.

Step 4 — complete the questionnaires. This is the part that matters. Your intake review begins when your questionnaires are submitted — not at the scheduled time. Complete them thoroughly: the depth of your answers directly determines how precisely Michael can read your pattern.

Step 5 — Michael reads your intake and your formula ships. Once your questionnaires are submitted, Michael reads the pattern, designs or adjusts the formula, and ships your herbs. If he needs clarification, he reaches out through the HIPAA-compliant messaging channel in your clinic account.

Reaching the provider.

You can message Michael directly from your clinic account at any time. But please understand: he cannot offer clinical guidance or speak to your condition until you have completed an initial intake. Just as a physician cannot advise on a patient they have never examined, herbal recommendations require a complete picture of your history and presentation first — the intake is what makes real guidance possible.
